I currently run ESXi 5.1 on a Gigabyte PH67-DS3-B3 motherboard and it works out of the box no problems. It does NOT have Intel VT-d and has NO on-board graphics card. However, I am looking into getting a new board which has Intel VT-d support.
According to Gigabyte, all UEFI boards have Intel VT-d. It really depends on what you are doing I guess. I want to pass hardware through to individual VM's so I need that capability.
Is this for a home project? What is your budget?

You will need to install the NIC drivers manually for these 2 boards, however.
There is an intel board which has 2 on-board NICs but cant remember model.
